We've seen all of these and a few others.

The #Name one appears to be specific to the Excel add-ins (rather than the Com add in) becoming unchecked. Checking them again has resolved the situation whenever we have seen this issue.

If you are using perspectives / architect then you might want to check that the option to see the processes is checked. The option is on the view menu.

Yup, not sure if it is a feature or a bug either, certainly its annoying. Our experience is that it only happens if you start with a slice / DBRW, remove some cells to do some pure excel work and then PAx adds back the DBRW. I assume its an attempt to provide some protection from users deleting form...
