Attached is a sample matrix that finally did the job of documenting security for a past project I delivered - it is admittedly "busy" but it seemed clear enough to hand over to the client's model administrator. Perhaps it will give you ideas you can use.

on (1) I would echo Marcus' comment and add that in the }Hierarchy Properties control cube you may also have to input a name for each level of each dimension before publishing in Framework Manager for the cube you are reporting from in Cognos. Excel was not the culprit in my case. I should cited it was Excel 2003 (with TM1 9.4 MR1) where I ran into the Process Worksheet issues. The data being loaded is not that extensive either, so the truncation issue was very unusual.
